3DWS Services Agreement

3-D WORLDVIEW SURVEY LLC SERVICES AGREEMENT

Effective Date: March 15, 2026

This Services Agreement (“Agreement”) governs participation in the 3-D Worldview Survey provided by 3-D Worldview Survey LLC (“3DWS”). By registering for or participating in the 3-D Worldview Survey, the participating school or organization (“School”) agrees to the terms of this Agreement.

  • Purpose

This Agreement governs the School’s participation in the 3-D Worldview Survey for the purpose of administering the survey instrument and receiving individual and aggregated reports intended to inform biblical worldview formation efforts.

  • Scope of Services

2.1 Administration Windows

Survey access includes the administration window(s) selected by the School for the designated academic year. Each administration window typically remains active for approximately two (2) weeks unless extended.

The School will provide an estimated participation count prior to each administration window. Additional administration windows may be added during the academic year subject to applicable fees.

2.2 Reports

The School will receive individual and aggregated reports based on participation levels. Reports are delivered electronically and are typically delivered within approximately fourteen (14) days of the close of each administration window.

2.3 Crosswalk Leader Hub

Access to Crosswalk Leader Hub, a digital leader hub providing guidance, tools, training, and instructional support resources, is included with participation in the 3-D Worldview Survey.

The School shall designate one Crosswalk Administrator. Up to four (4) additional named user accounts (five total accounts per School) may be included. All user activation or change requests must be submitted by the designated Administrator. 3DWS reserves the right to review and reasonably limit excessive user account changes.

Additional user accounts may be made available subject to applicable fees or written agreement.

Access remains active through the end of the designated academic year or for ninety (90) days following the final administration window, whichever is later. This limited post-administration access may extend beyond the academic year solely for this purpose.

2.4 Additional Services

Consulting, live training, or custom analysis services are not included under this Agreement but may be arranged separately.

  • Term

This Agreement applies to the academic year selected by the School during registration, as defined by the School’s instructional calendar.

Unused administration windows expire at the end of the designated academic year unless a rollover has been requested and approved.

  • Fees and Payment

Participation in the 3-D Worldview Survey requires payment of applicable registration and participation fees as described during the registration process or in related communications.

Fees are invoiced in accordance with the terms provided at the time of registration or participation. Payment terms are typically Net 30 unless otherwise specified.

  • School Responsibilities

5.1 Participation Estimates

The School will provide an estimated participant count prior to each administration window.

5.2 Compliance

The School is responsible for obtaining any necessary parental permissions and ensuring compliance with applicable student privacy laws and regulations.

5.3 Administration Integrity

The School will administer the survey under appropriate school supervision and will not distribute the survey link for unsupervised public access.

5.4 Permitted Internal Use

The School may provide survey access internally to its students, faculty, staff, parents, and board members for school-related use. The survey instrument and related materials may not be copied, reproduced, or distributed publicly except as permitted under Section 8.

  • Data Protection

Survey data is collected through a secure third-party survey platform and stored in secure cloud-based systems with access restricted to authorized personnel.

3DWS is not responsible for security incidents arising from third-party service providers beyond its reasonable control.

3DWS does not sell, share, or disclose student data to third parties except as required by law.

Data may be retained at the discretion of 3DWS for reporting and historical reference purposes. 3DWS does not guarantee indefinite archival availability.

  • Confidentiality

Both parties agree to maintain the confidentiality of non-public information received in connection with participation in the 3-D Worldview Survey. Confidentiality obligations survive termination or expiration of participation.

  • Intellectual Property

All survey instruments, scoring models, frameworks, reports, and Crosswalk materials remain the sole property of 3DWS.

3DWS grants the School a limited, non-transferable license to use reports and materials for internal educational and leadership purposes. The School may publicly share aggregated report graphics and summary findings provided that individual student data, survey questions, and proprietary scoring methodology are not reproduced.

  • Educational Nature and Limitation of Liability

The survey is an educational assessment tool and not a psychological, clinical, or therapeutic instrument.

To the fullest extent permitted by law, 3DWS’s total liability arising from participation in the 3-D Worldview Survey is limited to the total fees paid by the School for the relevant administration.

  • Termination

Either party may terminate participation at any time upon written notice.

Any applicable registration fees remain non-refundable. Fees for completed administration windows remain due. Access to Crosswalk Leader Hub may end upon termination, except where temporary post-administration access has already been provided.

  • Updates to this Agreement

3DWS may update this Agreement from time to time. The current version will always be posted at https://3dworldviewsurvey.com/services-agreement/. Material changes will be communicated to participating schools.

  • Entire Agreement

This Agreement constitutes the entire agreement governing participation in the 3-D Worldview Survey and supersedes prior discussions or understandings related to the services described herein. Participation in the survey and acceptance of this Agreement during registration constitutes agreement to these terms.
